Job Overview:

Our agency’s case managers work with healthcare providers and other professions in order to ensure that clients receive all of the care they need. Case managers are responsible for the entire patient care cycle, and they work to develop plans that maximize patient satisfaction while keeping costs manageable. Case managers assess clients based on professionals’ inputs and then design, implement, and monitor their patient care plans.

Responsibilities:

  • Reviewing records of new clients and conducting new-client orientations
  • Determining clients’ needs based on intake interviews and provided information
  • Recommending treatment programs that include regular routines and scheduled care
  • Monitoring client progress and verifying client attendance at appointments
  • Preparing client discharge documents and discharging clients when care plans are completed
  • Maintaining regular communication with clients and their families throughout the course of care
  • Addressing patient concerns with appropriate in-agency personnel
  • Updating client records on a regular basis
  • Reviewing financial trends of client care plans
